• Summary:  The goal of this funding opportunity is to expand access for community and faith-based organizations to implement community-level projects to improve infant and maternal health outcomes. These dollars seek to strengthen the unique work of community- and faith-based organizations (CBOs) as trusted partners of Ohio’s pregnant women and new families.
• Inquiry Dates:  Tuesday, November 14, 2023 – Monday, November 20, 2023.  All questions must be submitted via email to Hilary.Stoll@odh.ohio.gov and Stacy.Herman@odh.ohio.gov by Nov. 20, 2023. Questions received after this date will not receive a response.
• Letter of Intent Due Date: Wednesday, November 22, 2023, by 4 p.m.  A letter of intent and 501(c)(3) must be submitted via email to Hilary.Stoll@odh.ohio.gov, copying (cc:) Sara.Strauss@odh.ohio.gov by Nov. 22, 2023. A letter of intent must be on the agency letterhead and include the organization’s address, contact information, identify the counties that are being proposed to be served, and an approximate funding request amount. See Appendix E for Letter of Intent to Apply Template.
• Due Date:  Wednesday, November 29, 2023, by 4 p.m.  Applications and required components must be submitted via email to Hilary.Stoll@odh.ohio.gov, copying (cc:) Sara.Strauss@odh.ohio.gov by Nov. 29, 2023.  Each application component (A-C) must be clearly labeled and must follow the order listed in this Funding Opportunity.
