Summary:  Ohio Department of Health (ODH) is seeking applications providing support for local communities to build capacity for healthy eating and active living policy, systems, and environmental (PSE) changes. The objective of this project is to provide funding for community assessment and engagement with local community residents while working toward sustainability and expansion of these efforts within the county/jurisdiction.  Applicants who have never received HEAL or Creating Healthy Communities (CHC) awards are eligible for a HEAL Assessment & Photovoice award.
