The Curriculum Vitae form is required for the principal investigator and all other researchers who will be handling the data being used in a research protocol.  This applies to all data, whether obtained from existing data files or collected by the researchers in the course of your study.  In lieu of this form, you may provide one of the following:

1. An existing CV of 6 pages or less
2. A biographical sketch

We strongly recommend that all CVs and Confidentiality forms be scanned and sent as a single document.  This will speed processing time.
