This confidentiality agreement form should be used for all IRB protocols requesting ODH data EXCEPT requests for data from the Ohio Cancer Incidence Surveillance System (OCISS).  There is a separate confidentiality form for those requests.

The Principal Investigator and all researchers handling the data should submit a signed copy of this form.
