Ambulatory Surgical Facilities

Ambulatory surgical facilities provide same day outpatient surgery in a setting that functions separately from a hospital's inpatient surgical service or emergency department, and from the offices of private physicians, podiatrists, and dentists.

The Ohio Department of Health licenses and/or certifies approximately 250 Ambulatory Surgical Facilities /Ambulatory Surgical Centers


A licensed ambulatory surgical facility (ASF) provides outpatient surgery services.
A certified ambulatory surgical center (ASC) may be either hospital-operated or independent. If hospital-operated, the ASC must be a separately identified entity, physically and administratively distinct from other inpatient operations of the hospital.  An ASC may not provide a surgical procedure on a Medicare patient when, before surgery, an overnight hospital stay is anticipated. In cases where hospitalization after surgery is warranted, the ASC must be able to provide immediate transfer to a hospital. Ambulatory Surgical Facilities are licensed and regulated as Health Care Facilities  under Chapter 3701-83 of the Ohio Administrative Code.


All ASFs in Ohio are required to be licensed by the Ohio Department of Health under section 3702.30 of the Ohio Revised Code. The Bureau of Regulatory Operations is responsible for processing initial, renewal and change of operator license applications. The Bureau of Regulatory Operations, certifies ASCs for participation in the Medicare and Medicaid programs.  ASCs may obtain and maintain participation in the Medicare Program through accreditation by a CMS-approved accreditation organization.

The Bureau of Survey and Certification, is responsible for conducting on-site inspections/surveys for compliance with state and federal rules and regulations. ASCs are not currently subject to an annual survey visit, but do receive an unannounced inspection/survey on a schedule established by the Centers for Medicare and Medicaid Services.  During these surveys, all aspects of care and services are evaluated based on state and federal laws and rules.
Health Care Provider and Services: To obtain real-time information, generate, print and download reports regarding health care providers that are licensed and/or Medicare/Medicaid certified by the Ohio Department of Health.
