Health Care Services

The HCS program establishes the safety and quality of care standards for a provider of a health care service. The quality rules set minimum standards that a provider of the service must meet in order to offer the service.

The Health Care Service (HCS) program monitors the following services:

  • Solid organ transplantation

  • Bone marrow transplantation

  • Adult cardiac catheterization

  • Adult open heart surgery

  • Pediatric intensive care

  • Pediatric cardiac catheterization

  • Pediatric cardiovascular surgery

  • Operation of a linear accelerator

  • Operation of a gamma knife

  • Operation of cobalt radiation therapy unit

The HCS program establishes safety and quality of care standards for a provider of an HCS as listed above. The quality rules set minimum standards that a provider of the service must meet in order to offer the service. These rules are set forth in Chapter 3701-84 of the Ohio Administrative Code.

In 2016 the Ohio Department of Health established new service levels, operating standards, and reporting requirements for adult cardiac catheterization laboratories in Ohio. More information and forms for Adult Cardiac Catheterization Services may be obtained from our Adult Cardiac Catheterization  page.

More information and forms for Adult Open Heart Surgery Services may be obtained from our Adult Open Heart Surgery Services page.

Process:

Notification to the director must be filed by a prospective service provider at least 30 days prior to initiating a new HCS or reactivating a discontinued or temporarily suspended HCS. The written notice must contain the name, address and telephone number of the facility where the HCS will be located, the type of HCS to be provided and the projected date of initiation of the HCS. An acknowledgement of receipt of the notification will be sent by the Ohio Department of Health (ODH), Bureau of Regulatory Operations, with an HCS Notification Survey and Attestation of Compliance, which must be completed and returned to ODH prior to initiating the service. A revised Attestation of Compliance must be filed within 30 days of a change in the personnel signing a previously filed attestation. The program ensures compliance with the quality rules through self attestation of compliance by the service provider and site visits. You may obtain these documents below:

Enforcement:

Section 3701-84-05 of the Ohio Administrative Code provides for the Director of Health, upon discovery of a failure to comply with any requirement in the quality rules, to provide a reasonable amount of time to correct the violation, or impose a civil money penalty based on the severity of the violation.
