This agreement gives local health departments the ability to set up and manage new user accounts within their jurisdiction. The local health department (LHD) administrator agreement should be signed by all new ODRS administrators and the health commissioner.  The administrator agreement should then be returned to ODH and the ODRS administrator will create the new user account.
