An application for permit must be submitted to the local health district to construct, alter, or seal a private water systems, test well, or part thereof as required in Ohio Administrative Code (OAC) 3701-28-03(A).  The application must be submitted complete and include the site plan form.

The site plan form must be complete including a drawing of the property showing the location of the private water systems with distances from possible contamination sources. 

No work shall begin on the construction or alteration of a private water systems before the permit application has been validated by the local health district, with the exception of an emergency construction or alteration as addressed in OAC 3701-28-03(J).
