BRFSS Prevalence & Trends Data – Tool for viewing and downloading prevalence estimates from the Behavioral Risk Factor Surveillance System (BRFSS) that includes charts, graphs, and maps at the state and national level. https://www.cdc.gov/brfss/brfssprevalence/index.html

500 Cities: Local Data for Better Health – The 500 Cities Project provides city- and census tract-level small area estimates for chronic disease risk factors, health outcomes and clinical preventive service use for the largest 500 cities in the United States. https://www.cdc.gov/500cities/

CDC Chronic Disease Indicators – Set of surveillance indicators that enables public health professionals and policymakers to retrieve uniformly defined state and selected metropolitan-level data for chronic diseases and risk factors that have a substantial impact on public health.  https://www.cdc.gov/cdi/

CDC National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) – The NCCDPHP works to prevent and control chronic diseases through community and health system interventions, especially for groups affected by health disparities. This website includes information on their programs, chronic disease data, and tools and resources. https://www.cdc.gov/chronicdisease/

CDC WONDER – Provides online databases that utilize a rich ad-hoc query system for the analysis of public health data. Reports and other query systems are also available. https://wonder.cdc.gov/   

County Health Rankings & Roadmaps – The annual County Health Rankings measure vital health factors in nearly every county in America and provide a revealing snapshot of how health is influenced by where we live, learn, work and play.
