Private Water System Site Plan – Additional Plans

As required in rule Ohio Administrative Code (OAC) 3701-28-03(F), additional plans must be submitted to the local health district as part of the permit application when:

  • Constructing or altering a private water system servicing a building as defined in section 3781.06 of the Ohio Revised Code.
  • Constructing or altering a private water system servicing a structure that is not a one, two, or three family dwelling.
  • Constructing or altering a private water system using a pond, cistern, or spring as a source of water.
  • Constructing or altering a private water system using a well drilled in an area of known flowing well conditions.
  • The private water system will be located within 300 feet of a land application area.
  • Installing a drive point well.
  • Installing a buried pressure tank.
  • Installing a gasoline powered pump and pressurized system.
  • Installing a continuous disinfection and/or filtration system.
  • Installing a point of entry water treatment system intended to remove or reduce a contaminant to below health based standards.

Plans may be required to be submitted in duplicate, by the local health department, when plans are not submitted electronically.

The required plans must meet the requirements in OAC 3701-28-03(G).  The forms on the right side of the page are available to assist the private water systems contractor in submitting the proper information to the local health department.
