Only the facility's Individual Responsible for Radiation Protection (IRRP) is granted access by Ohio Department of Health (ODH) to make online record changes.
The IRRP must have an established user ID in the ODH Application Gateway's secure system. To change the IRRP on record, complete the IRRP Change form HEA5526 and send to email@example.com or fax to 614.644.8526. For facilities with more than one location, submit all registration numbers on the same form.
We can only use one e-mail address per IRRP and that e-mail can only be used for one individual.
Individual Responsible for Radiation Protection (IRRP) changes are not Responsible Officer changes.
The differences between the Individual Responsible for Radiation Protection (IRRP) and the Responsible Officer are as follows:
Individual Responsible for Radiation Protection (IRRP): The individual designated as responsible and readily available for radiation protection and implementing quality assurance policy and procedures necessary for assuring compliance with Chapter 3748. of Ohio Revised Code and rules adopted thereunder. Responsible Officer: The president, vice-president or other individual in the organization of a corporation, partnership, or other entity who is vested with executive authority over activities subject to this rule, or an individual who is appointed or elected according to law, who is authorized to manage and direct the affairs of a corporation, partnership or other entity. In the case of an individual proprietorship, responsible officer means the individual.
