2025 Vaccines for Children (VFC) Annual Recertification

All currently enrolled Vaccines for Children (VFC) providers are required to submit a 2025 VFC Provider Agreement and Provider Profile to complete VFC recertification. The required forms are located on the Ohio Department of Health’s website. The forms must be completed and signed by the provider site’s medical director. Completed forms should be submitted to ODH no later than Friday, Feb. 28, 2025. Failure to submit completed forms by the deadline may affect future vaccine ordering for a VFC provider office.

VFC coordinators at each location are required to complete training at least once every 12 months. The required 2025 training is “You Call the Shots” module 10 (Storage and Handling) and module 16 (Vaccines for Children Program). VFC coordinators who have successfully completed these modules since 2023 may complete a refresher test for each topic. Links to the trainings are below.

VFC coordinators should maintain the certificates of completion on site to be reviewed during site visits. Unless individually requested, certificates should not be submitted with recertification forms.

The completed and signed VFC Provider Agreement and VFC Provider Profile forms can be submitted to the Ohio Department of Health Immunization Program via any of the following methods:

  1. Email the scanned forms to: vfc@odh.ohio.gov.
  2. Fax the forms to (614) 728-4279 Attention: VFC Program.
  3. Mail the completed original forms to:

VFC program communications will be sent out electronically from vfc@odh.ohio.gov. Therefore, it is imperative that valid email addresses for the VFC coordinators are provided on the submitted forms. 

A confirmation of receipt email will be sent to the VFC coordinators listed on your site’s submitted 2025 VFC forms.
