All Vaccines for Children (VFC) providers must use the Ohio VFC vaccine management plan template to document vaccine handling and storage processes. The template contains all VFC program required elements for a management plan. It should be maintained at a location accessible to all staff that handle vaccines and near the vaccine storage units.  The vaccine management plan should be reviewed and updated annually or more frequently as staff changes.
