About CMH Metabolic Formula Program
The Metabolic Formula Program under the Complex Medical Help Program (CMH) provides metabolic formula and essential amino acids to individuals born with inborn errors of metabolism that are screened for the Ohio Newborn Bloodspot Screening Panel. Examples of these disorders include phenyketonuria (PKU), maple syrup urine disease (MSUD), homocystinuria (HCU), tyrosinemia, and propionic acidemia. Without these special formulas, individuals, especially infants and young children, may develop poor health outcomes including developmental delays and possible death. Coverage of metabolic formula can continue for any Ohio resident, regardless of age or income, with a qualifying diagnosis who is followed at an Ohio Regional Comprehensive Genetics Center.
ODH will order the prescribed metabolic formula and have it shipped directly to the patients' homes. Formula is provided at no cost to the patient. Funds to support the program come from a portion of the newborn screening fee and the CMH program. ODH provides metabolic formula to over 300 individuals in Ohio.

Disorders Covered by Metabolic Formula Program
The Ohio Metabolic Formula Program Diagnosis Eligibility:
All inborn errors of metabolism flagged on the Ohio Newborn Bloodspot Screen Panel that are then confirmed with follow-up testing and diagnostics, which require metabolic formula, will be eligible for the program. Requirements for participating in the Metabolic Formula Program
To participate in the metabolic formula program, an individual* must:
- Establish and maintain permanent Ohio residency
- Receive care for the eligible metabolic disorder at an Ohio Regional Comprehensive Genetics Center
- Maintain follow-up visits annually (at minimum) with CMH-approved geneticist
*For individuals eligible for Ohio Medicaid, the Medicaid program typically covers the cost of metabolic formula. Contact Medicaid directly for assistance and enrollment.
