Advancement Application - Verification of Work Experience

This downloadable form is used by SITs applying for advancement from SIT to RS. This form must be completed by both the SIT and their supervising RS.  The SIT must also list, in detail, the duties they performed that constitute the practice of environmental health that qualify them for advancement.
