Print Certificate of Registration

A Certificate of Registration confirms all radiation-generating equipment for a registered address(s) and must be displayed/posted at each facility.

If adding additional equipment to your registration, a new application is not required.

Registration Certificates are not transferrable.

A Certificate can be printed online if the facility's record has a zero balance.

Ohio Department of Health approved amendments and invoice payments have a waiting period for processing before the new certificate will be available for printing.
