Consumer Confidence Reports
Consumer Confidence Reports (CCRs) must be prepared by all community public water systems and directly distributed to customers by July 1 annually. Public water systems that sell water to community public water systems must provide information that is necessary to complete their CCRs to the communities by April 1 annually. Please consult Ohio Administrative Code (OAC) Chapter 3745-96 for the rules associated with CCRs.
CCR Template, Instructions and Forms
If you are preparing a CCR for the first time, start with the CCR Template. The template contains all of the required elements of the CCR, including mandatory language. You must include all sections that are relevant to your PWS, but you should remove references to situations that do not apply. For example, a ground water system does not have to include turbidity language should delete that information from their report. The Template does not address all situations, but the Instructions manual explains each corresponding section of the Template, and provides examples beyond what is contained in the Template. The Instruction Manual does not repeat all of the mandatory language contained in the Template, so they are designed to be used together in order to have a complete report.
Ohio EPA has updated our CCR Delivery Certification Form and our CCR Instructions to include electronic delivery information for this year.
As of April 1, 2024, Ohio EPA will no longer provide the courtesy draft review service for CCRs that has been done in past years. However, the CCR Team will continue to provide guidance and answer specific questions.

CCR Electronic Delivery Options
Electronic delivery is one instrument that public water systems can use in order to meet the requirement to directly deliver their CCR to each customer.
For Electronic delivery the notification must reach all consumers and include:
- A notice that the Consumer Confidence Report or Annual Water Quality Report is available
- A direct link to the viewable CCR on the web page (must lead directly to readable report)
- A contact method to request a paper copy (i.e., a phone number or email address)

Table of Detected Contaminants

The Table of Detected Contaminants information provided by Ohio EPA is not intended to be complete or all inclusive. The tables are intended to assist public water systems with their data collection and consolidation efforts. Public water systems will need to verify and add additional information to ensure their Table of Detected Contaminants is complete as required by the Consumer Confidence Rule. For a blank template for a Table of Detected Contaminants and for assistance calculating averages, please see the “Tools and Calculators” tab above.
- All tables must include chlorine. Please ensure chlorine is in the table and is correct.
- Individual Lead results over 15 ppb must be added to the table (i.e., If 2 samples are over 15 ppb, at 22 ppb and 28 ppb, you must include both values in the table)
- Surface Water Systems must include turbidity levels, the percentage meeting the standard, and total organic carbon (TOC)
- Purchased water systems must provide entry point data from their suppliers. This information is required to be sent by the parent water supply by April 1st. Purchased water systems must also include their own distribution sampling results in the table (i.e., copper, lead, DBPs, chlorine)
- Water systems that have multiple entry points to their distribution system will need to have data from each entry point listed.
- Occasionally raw water data may appear in the auto-generated Excel file. Raw water data should not be included in your final table.
- If the data for “year sampled” is blank, the year should be entered as the year in which you are writing the CCR report for.

For PWSs with a population of 500 or less, Ohio EPA has posted the data that should be contained in the 2024 CCR Tables of Detected Contaminants. If you have questions regarding your 2024 data, or need help generating your Table of Detected Contaminants, please email CCR@epa.ohio.gov.
To find your water system’s draft CCR table information, click the link below and use the search criteria of either your PWS ID or the PWS name. Once the data is retrieved, you can download an Excel file of your results by clicking the Excel button above the table.
