Annual Required Assessment Reporting for Schools - Kindergarten Through 12th Grade.
Ohio Revised Code section 3313.67 requires schools (K-12th) to report the immunization status of students by October 15 of each year. Each school or portion of a school that has its own Information Retrieval Number (IRN) must submit a separate report for all applicable grades.

If you need help locating your child’s vaccination records, you can:
- Ask your child’s healthcare provider, who will have a copy of their medical records and may be able to access the state system.
- Contact your local health department, which may be able to access the state system.
- Ask a workplace, camp, or school that you may have provided the records to in the past.
- Complete and mail in an Ohio Department of Health Authorization to Disclose Form.
- The Ohio Department of Health maintains the Ohio Impact Statewide Immunization Information System (ImpactSIIS), an online tool that allows providers to record immunizations/vaccinations administered in the state of Ohio. If you are seeking to obtain a copy of your child’s immunization records from the state system, it is important to know that such records are protected by medical confidentiality laws.
- Medical providers in Ohio are not required to report immunizations to the state system. It is possible that other vaccinations will not be recorded in our system.

CDC Coverage Definitions for Kindergarten.
- Greater than or equal to four DTaP: Four or more doses of diphtheria, tetanus, and pertussis (DTaP) vaccine. If all four doses were given before the fourth birthday, a fifth dose is required.
- Greater than or equal to three Hep B: Three or more doses of hepatitis B (hep B) vaccine.
- Greater than or equal to two MMR: Two doses of measles, mumps, and rubella (MMR) vaccine.
- Greater than or equal to three Polio: Three or more doses of inactivated polio vaccine (IPV).
- Oral polio vaccine given after April 1, 2016 should not be counted as a valid dose.
- Greater than or equal to two Varicella (chickenpox): Two doses of varicella vaccine.
- Medical exemption: a medical exemption is allowed when a child has a medical condition that prevents them from receiving a vaccine.
- Non-medical exemption: a vaccine exemption for religious or philosophical reasons.
- Ohio non-medical: Ohio law defines as reasons of conscience, including religious convictions.
