Licensure - General Information and Fees

General Information

Radiologic licenses are issued for the following categories: Radiographer, Nuclear Medicine Technologist, Radiation Therapist and General X-ray Machine Operator (GXMO). Please click on each link below for specific requirements for each category.

• Radiographer, Nuclear Medicine Technologist and Radiation Therapist license requirements 
• General X-ray Machine Operator license requirements 

Fees

• Initial application fee: $65 
• Renewal application fee: $45
• Current GXMO licensee converting to a Radiographer license: $15
  Note: The Radiographer license will have the same expiration date as the current GXMO license.

**All fees are non-refundable and non-transferable; acknowledgement is affirmed when signing the "Statement of Declaration" section on the application.
