Reporting & Forms

Surveillance

Surveillance is a core public health function that includes reporting of persons suspected of having or having an infection or disease required to be reported to local and state public health, maintaining a data system of those cases and suspected cases, analyzing data, and generating summary reports for local health jurisdictions, health care providers, community organizations, and funding sources.  Maintaining complete and accurate data on cases and suspected cases is necessary for understanding the epidemiology of hepatitis B (HBV) and hepatitis C (HCV) and is essential in detecting outbreaks in Ohio.

Reporting

In Ohio, healthcare providers and facilities are required per Ohio Administrative Code 3701-3-02 to report the occurrence of cases of suspected cases of HBV or HCV infections under their care and treatment to the public health authority.  Hepatitis B and C should be reported to health authorities by the end of the next business day.  Provider and laboratory reports should be made to the local health department in which the patient resides.
